Kaitlyn Cole

Kaitlyn Cole is the program manager for the UW Health Hub and Spoke Program which helps patients with opioid use disorder get connected with care and medication. She completed her BSN from the University of Wisconsin-Eau Claire. She started out as a nurse resident at the University of Wisconsin Hospital and Clinics Inpatient Psychiatry Unit. She then went on to get her Master of Science in Nursing and Master of Health Care Administration through the University of Phoenix.

She spent 6 years as a nurse case manager with the UW Health Complex Case Management Program, focusing on meeting the needs of patients with various medical and psychosocial complexities, helping to connect all pieces of the health care puzzle, bringing the various disciplines together to communicate, treating patients as a whole person, and not just based on their diagnosis. She is a Certified Case Manager (CCM) and a member of the Case Management Society of America. She has a passion for helping patients get connected to care by eliminating barriers and meeting patients where they are at in their journey.
